About this grant

ACL plans to award cooperative agreements to develop and expand dementia-capable community health worker (CHW) programs within the National Aging Network. Projects will integrate CHWs into community-based aging services to improve dementia screening and referral, deliver dementia-specific evidence-based or evidence-informed interventions, reduce barriers to care, and improve access to supportive services and quality of life for people living with dementia and their caregivers.

Who can apply, from the official notice

must be an Area Agency on Aging (AAA) or demonstrate active participation in local aging services network and partnership with AAA in project service area Foreign entities are not eligible to compete for, or receive, awards made under this announcement. Faith-based and community organizations that meet the eligibility requirements are eligible to receive awards under this funding opportunity announcement.
